Verifying coverage is the responsibility of the client.
Please call your insurance company before our first session to confirm your benefits. Be sure to ask the following questions:
I am considered an “out of network” provider for all other insurance companies. If you choose to use another insurance company, you must pay in full at the time of our appointment. I can then provide you with an invoice to be submitted to your insurance agency in order to reimburse you some portion of that fee. Please confirm with your insurance company that this service will be available to you before our first session.
All payments are taken at or before each appointment. In the event that you are unable to keep an appointment, please notify me via phone a minimum of 24 hours in advance. If you miss your appointment for whatever reason or fail to give adequate notice, you will be responsible for the full fee of the session.
